The Program of All-Inclusive Care for the Elderly, an American health care program, provides comprehensive health services for the frail elderly. CenterLight Teamcare
CenterLight Healthcare provides a comprehensive, long-term care program called CenterLight Teamcare. This program provides a customized plan that includes medical and home care as well as social activities and transportation. This model of care has been a proven way to reduce avoidable hospitalization and improve health and quality of life for the elderly. The CenterLight Groupcare plan is available for senior residents who require assisted living and residential care.
WelbeHealth
WelbeHealth is just one of many senior care providers across the country. The company offers comprehensive medical, therapy, social, and other care to older people in their community as part of the PACE Program. This program allows individuals to maintain their independence and remain in their own homes. WelbeHealth is committed to helping people age in their own places. It is California's largest provider of PACE.
On Lok
On Lok expands its program by incorporating a social day care center into the On Lok model. It also offers elderly persons housing assistance and in-home services such as home-delivered meals and home-delivered care. These services extend the On Lok model of care from providing basic medical care to providing social support to older adults. On Lok has received a federal grant by the Department of Health and Human Services that allows it to grow and create a national model of comprehensive care for the older. On Lok funds the creation of the National PACE Association. Now there are 21 PACE programs across nine states.
PACE Southeast Michigan
The PACE Southeast Michigan program offers all-inclusive care that provides a continuum to seniors who want to live in their own homes. These services include counseling, therapy, meals, and physical therapy. They also offer outpatient and behavioral health services as well as substance abuse and addiction care. PACE Southeast Michigan also provides transportation and home maintenance services.
WelbeHealth’s model of care
WelbeHealth's all-inclusive senior care model is not new. The organization has been providing senior services for many years, and was praised in the midst of the deadly influenza pandemic last year. A study from the University of California, Berkeley found that the death rate of WelbeHealth patients was 5 times lower than that of nursing homes and other PACE organizations. These programs help California's most economically disadvantaged areas.
Medicare and Medicaid reimbursements
PACE Programs, which are pre-paid capitated plans for the elderly, are intended to provide all Medicare and Medicaid-covered services in one comprehensive plan. Providers must provide equal care to all Medicare eligible clients and Medicaid-eligible patients to be eligible for this program. PACE reimbursements to Medicare and Medicaid are based upon prospective capitation. A facility must be in good standing both with the federal and state health insurance programs to be considered a PAE provider.
