CenterLight Teamcare
CenterLight Healthcare provides a comprehensive, long-term care program called CenterLight Teamcare. The personalized plan includes both home and medical care. There are also social activities, transportation, congregate meals, and other services. This method of care is proven to reduce unnecessary hospitalizations and improve the health and quality life of seniors. There are many CenterLight Teamcare plans that can assist seniors, whether they need residential or assisted living.
WelbeHealth
WelbeHealth is one of hundreds of senior care providers in the country. The company provides social, medical, and therapy services to elderly people living in poverty as part its PACE program. Individuals are encouraged to remain independent and continue living safely at home. WelbeHealth's mission aims to support people as they age in their homes. It is the largest provider of PACE services in California.

Lok
On Lok now offers a social service center as part of its On Lok program. It also offers home-delivered meals, in-home assistance, and housing assistance to seniors. These services go beyond the On Lok model and offer social support as well as basic medical care. On Lok receives a federal grant through the Department of Health and Human Services to help it grow and develop a national model of care for the aged. The National PACE Association is established with On Lok's support and there are now 21 PACE programs operating in nine states.
PACE Southeast Michigan
The PACE Southeast Michigan program provides all-inclusive care. This includes a variety of services to assist seniors in living independently at home. These services include counseling, therapy, meals, and physical therapy. They also provide outpatient services and prescriptions. PACE Southeast Michigan provides services that include transportation, recreation, and home maintenance.
WelbeHealth's model of care
WelbeHealth's concept of all-inclusive elder care is not a new idea. The organization has been providing senior care for many years. They were praised last year during the influenza pandemic. University of California Berkeley's study found that WelbeHealth patients had a death rate five times lower then that of nursing home residents and other PACE groups. These programs are for the most economically poor counties of California.
